New Patient Information

During your comprehensive exam, which will last approximately 1-1 ½ hours, you will be screened by an ophthalmic technician who will ask you questions about your eyes, physical history, and general health.

 

A series of eye tests will be performed which may require eye drops to dilate your pupils. After dilation, some patients prefer not to drive. In that case, please prearrange for a ride home. Most patients, however, find they are able to drive comfortably after the exam – although closeup vision is affected temporarily. To make your vision more comfortable after dilation, you can bring dark glasses or use the disposable lenses we provide.

 

Save Time: If you are a new patient to our office, you may save time by completing our New Patient Forms prior to arriving for your appointment.

 

The process is easy as 1-2-3:

1. Click to open, then print the form.

2. Complete the forms at a time convenient for you.

3. Bring the forms along with your insurance information to your first appointment.

 

Insurance / Financial Information: As a courtesy, if you belong to one of our participating insurance providers, we can bill your insurance on your behalf if you provide us with the proper information. Please bring your insurance cards with you to your visit. Medical insurance plans are private contracts between patients and their insurers. While we will bill your insurance on your behalf, you are ultimately responsible for payment of your bill. If you have a managed care insurance plan, you are responsible for obtaining any referrals the plan requires, with the exception of surgery referrals. You should contact your insurance company directly with any questions about coverage/providers that are in-network.  

 

Due at Visit Costs: Co-payments and payments for any services not covered by insurance are due at the time of your visit. If you have an unmet deductible we will require a deposit of $150 at the time of service.  

 

Cancellations: If you need to cancel your appointment, we will gladly reschedule to a more convenient time. Please try to give at least 24 hours notice. In case of emergency, call either location at any time to reach our full-time answering service.
